Role Description

This is a full-time remote position for a Health Case Manager. The Health Case Manager will coordinate patient care, provide education and resources, and ensure efficient delivery of healthcare services. They will monitor patient progress, communicate with healthcare providers, and advocate for patient needs. Responsibilities include maintaining patient records, developing care plans, and adhering to regulatory and compliance requirements to uphold the highest standards in patient care.

Qualifications

• Case management and care coordination skills, including the ability to advocate for patients and create tailored care plans.
• Knowledge of healthcare systems, medical terminology, and regulatory compliance.
• Strong communication and interpersonal skills to interact effectively with patients, families, and healthcare providers.
• Organizational and documentation skills to manage patient records and track progress accurately.
• Problem-solving abilities with a compassionate, patient-focused mindset.
• Experience with remote work and proficiency with relevant technology platforms and software.
• Bachelor’s degree in a related field (e.g., Nursing, Social Work, or Healthcare Administration). Certified Case Manager (CCM) credential is a plus.
